This form has thirteen sections. The first question is Overall, how satisfied were you with the product / service? The options are Very Satisfied, Satisfied, Unsatisfied, Neutral and N/A. these are the options your users are shown to choose from

The next question asks to user How long have you used our product / service? Option are - Less than a month, 1-6 months, 1-3 years, Over 3 Years and Never used. Customers can choose any one required option.

Next section asks to customer How often do you use product / service? Option are Once a week, 2 to 3 times a month, Once a month and Less than once a month. This options .is required. Choose any one option.

Next section asks to customer to What impressed you most about the product / service ? Options are Quality, Price, Shopping Experience, Installation or First Use Experience, Usability and Customer Service. Customer choose any required option.

Next section asks to customer What disappointed you most about the product / service ? Options are Quality, Price, Shopping Experience, Installation or First Use Experience, Usability and Customer Service. Customer can choose any one of these, this is also required option.

Next section asks to customer What do you like about the product / service? Customers may write their own opinion as an answer to this question.

Next section asks to customer What do you dislike about the product / service?. This section is also customer opinion, customer write reason why he was dislike the product in sort description.

Next section asks Would you use our product / service in the future? Options are Definitely, Probably, Not Sure, Probably Not, Option 5 and Definitely Not. This is required option. Customer can choose any one option.

Next section asks Would you recommend our product / service to colleagues or contacts within your industry? Option are Definitely, Probably, Not Sure, Probably Not and Definitely Not. This is also required option choose customer any one.

Next section asks How often do you use product / service? Options are Once a week, 2 to 3 times a month, Once a month and Less than once a month. This is also required.

Last section asks to the customer. This is a required question. Thinking of similar products / services offered by other companies, how would you compare the product / service offered by our company? Option are - Much Better, Somewhat Better, About the Same, Somewhat Worse, Much Worse and Don't Know. Customer can choose any one option.
