The second section asks What is your gender? Option are Male, Female and Other. choose any one of the required options of these.

The next section asks What is your age? The Options are Under 18, 18-24, 25-34, 35-44, 45-54, 55-64, 65 or Above, Prefer Not to Answer and Other.

Next section asks What browser do you use? This is a required question. Options are shown Google Chrome, Firefox, Netscape, Safari, Internet Explorer, Opera, AOL, Don't know and Other. Users are choose any one option.

Next section asks What is the speed of your Internet connection? The options are shown Cable/ISDN, Dial-up, T1, DSL, T3, 33.6 or less, 56K, Don't know, and Other. Choose any one required options.

Next section asks to user Do you follow us? This is a required question Options are By RSS feed, By Newsletter, By Facebook, By Twitter and Other. Choose the option any one.

Next section asks to user How often do you visit our site? Options are Daily, 3 or 4 times a week, Once a week, 2-3 times a month, Monthly, Less than monthly, Not sure and Other. Choose any one required option.

Next section asks Why did you visit our site today? Options are To find a link to something, Trying to buy something, Searching for specific, content information, Looking for general information, Find contact or company information, Entertainment, Tutorial support and Other. Choose any one required option.

Next section asks What is your total annual household income? Options are Under $20,000, $20,000 - $30,000, $30,000 - $40,000, $40,000 - $50,000, $50,000 - $75,000, $75,000 - $100,000, $100,000 - $150,000, $150,000 or more, Prefer Not to Answer and Other. Choose any one required option.

Last section asks Please add any comments. This is a required question. Users may write their own opinion as an answer to this question.
